Acceptance of Terms
By enrolling in the HearingLife informational and conversational SMS program, you agree to these Terms of Service. If you do not agree, please do not sign up or reply STOP to Opt-Out at any time.
Description of Messages
By providing consent, you may receive text messages,
which may include updates from HearingLife at the
number you have provided.
To Opt-in to SMS Messages- Your written consent to receive SMS messages will be collected on the Patient Intake Form.
Message Frequency: Varies per month. Standard message and data rates may apply based on your carrier’s plan.
Information We Collect
When you opt-in to receive SMS messages, we may collect the following:
Phone Number
Name
Address
Email Address
Insurance Information
Interaction Data - Such as message delivery status and responses.
We do not store sensitive information or Protected Health Information (PHI) through SMS.

You can request help by replying HELP to any SMS Message

Once you Opt-Out, we will no longer send messages but may retain records for compliance purposes.
Privacy and Security and Sharing Information
We value your privacy and will NOT sell, rent, or share your information with third parties for promotional or marketing purposes. However, we may share your information with our subsidiaries and affiliates, service providers who help manage our SMS program, or with legal authorities as required by law.
SMS messages are not encrypted and there is a risk that messages could be intercepted. For sensitive healthcare matters, please contact a HearingLife office directly via telephone or the HearingLife headquarters at 888-894-7847.
Do Not Call Registry and Consent
By submitting, you acknowledge that your consent supersedes any registration on the National Do Not Call Registry. You understand that participation in receiving SMS messages is voluntary and not required for you to receive medical treatment.
Changes to the Terms of Service
We may update these Terms at any time. Continued participation in our SMS program after changes take effect constitutes acceptance of the new Terms. The latest version will be posted on our website.
